When we talk about harm to others, we mean having thoughts about seriously hurting someone else, making plans to do so, or feeling like you might lose control and act on those thoughts.
-

The ROI lets you decide if I can communicate with other people involved in your care—like a doctor, psychiatrist, school, or family member. Without your written permission, I don’t share your information.
You’re in control of what gets shared. The form can be changed or canceled it at any time.
If you have any other concerns regarding confidentiality, mandated reporting, or a special situation, please feel free to contact me and I will be more than happy to help out.
